Registration Group 1: January – 7 June 2011
Registration Group 2: January – 21 June 2011 - Admission fee for each participant for two weeks course is 1850 USD*
*1 USD = 9500 IDR

- 2 (Two) latest photos size 4x6
- Copy of birth certificate
- Medical Letter of fitness to physical activity (letter from school physician or family doctor)

Indonesia. - Successful applicants will be notified by email and will be contacted by phone. Receipt will be issued to successful applications.
- Places for all courses are strictly limited and will be allocated on a first -time first served basis.
- All Classes will be conducted mainly in Bahasa Indonesia and English.
- Application for registration by phone or fax will not be accepted.
- Proof of age must be provided upon request in the registration form
- All courses, activities, venues and arrangements are subject to change according to weather, programming, a satisfactory level of numbers and others factor beyond MJC or brand holder and/or its affiliates’ control. We reserve the right to cancel or change any courses, should this be necessary for any reason.
- No catch up lesson will be rearranged for any participants who are ill or are absent from class for personal reason.
- You agree to grant MJC the world wide right in perpetuity without approval or compensation, to use their child’s name, photographic or video image or likeness for MJC’s purposes including friendly match, trip to Italy, training in AC Milan italy for “All Star Team Challenge” winners.
- MJC treat as a priority the safety and wellbeing of all children attending our courses. We therefore reserve the right to remove from our courses without refund, any child who is found bullying, misbehaving or generally disruptive.
