1. Download registration form through www.acmilanindonesia.com or get the printed form from our office & partners (ASD office, Tabloid Bola, Milanisti Indonesia)
2. Admission fee for Milan Junior Camp Bali is *1850 US$ per 2 (two) weeks program.
   *1 US$ = 9500 rupiah
3. Choose either:
   Group 1: June 18th to July 2nd 2011 (limited to 75 kids)
   Registration Period: January – 7 June 2011
   Latest Payment: 7 June 2011
   Group 2: July 3rd – July 17th 2011 (limited to 75 kids)
   Registration Period: January – 21 June 2011
   Latest Payment: 21 June 2011
   *Early bird discounts are available for payments received prior to 31 December 2010

5. Complete the registration form & sign the parental consent pages.
6. Complete the following documents:
   - Latest photos size 4cm x 6cm
   - Medical certificate of fitness to physical activity (letter from school physician or family doctor)
   - Copy of ID or Birth Certificate of the participant & the legal guardian
   - Copy of payment receipt
7. Mail the completed registration form and all documents above to :
